Helpful Hints to make sure your intake form is completed: 


1. Add information to the Short Description of your Services: Go to Services -> open the Services -> add to the Short Description -> Save 


This is what it will look like on your online booking page. 




2. Modify the Subject and Body of your booking and reminder emails: Go to Business Setup --> Your Clinic --> Message Settings -> select the booking and reminder emails for your services with forms -> edit the subject and message body (for example as below) -> Save 




3. Create a reminder email template: Go to Content --> Content Library --> Add --> add text similar to the below and include the link to the form --> Save



To use this as an email --> go to the patient record --> select Send --> Email --> select this block from the Content dropdown --> OK.
